Greetings SCS Families,

My name is Annie Horn and I am the school nurse on campus. I have been a nurse for sixteen plus years in various roles and am now here to keep our students and staff physically, emotionally, and spiritually healthy throughout the school days. I am located in the Daniel Building, and work alongside Renee Lange in the Early Childhood Education Center (EEC) to provide clinical assessment and interventions for student and staff needs as they arise. Other responsibilities of the health offices include immunization compliance, wellness campaigns, complex care planning and collaboration, and student education. Please do not hesitate to reach out with questions or concerns regarding your student’s health.

Health Records

School Procedures for Illness or Injury

Students who become ill or injured during the school day should obtain a pass from their teacher to go to the health care office. If the student must be sent home, the health care representative will call and ask the parent or guardian to pick up the child. If it is a high school student who drives to school, the health care representative will call the parent or guardian to obtain permission for the student to leave school. If a determination is made that paramedic assistance is necessary, 9-1-1 will be called and the child will be taken to a nearby hospital. Each village sets its own fee for ambulance transportation.

Any student with a temperature of 100 degrees or above, or who has experienced vomiting or diarrhea within the previous 24 hours, will not be allowed to remain in school. The parents will be contacted and asked that the child be taken home or to a doctor. The student must be symptom free for 24 hours without medications before returning to school.

Emergency Treatment Release/Medication Authorization

At the beginning of the school year all parents or guardians are required to complete an Emergency Treatment Release/Medication Authorization Form. One form is required for each student in attendance at Schaumburg Christian School. These forms are sent out by email once you are enrolled and are to be completed electronically.

It is important that the emergency contact phone numbers for your family stay current. If there has been a change in a mobile phone number, a job change, or a change in emergency contacts, please complete the Information Update Form and submit it to the school office. You may pick up this form in the school office or call the office to update your information.

Participation in Physical Education or Recess

As a general rule, if a student is well enough to attend school, he is well enough to attend physical education classes and recess. With written parental verification, a child may be excused from physical education or outdoor activities for up to two days following an illness or injury. If a student is unable to participate in a regular physical education class or recess for more than two days, the health care office must be given a medical excuse from a physician stating the nature and time limitation of the condition. All PE excuses from parents/guardians need to go through the nurse's office.

Immunizations

All immunizations must be current according to the requirements of the State of Illinois. Students who fail to comply with these requirements may not be allowed to attend to school. Transfer students are expected to meet the same requirements.

Students entering an Illinois school for the first time (home-school and out-of-state transfer) must submit evidence of a vision examination, completed by a doctor specializing in diseases of the eye or a licensed optometrist, dated within the past 12 months.
Proof of this examination is due by October 15.
Transfer students who are admitted and enrolled after October 15 of the current school year shall have thirty days following registration to comply with these health examination, immunization, dental examination, and eye examination requirements, as applicable

Grade Based Requirements:

NEW students to SCS require a physical examination with immunization records in compliance with their current grade level, a dental exam and vision exam. If a student is moving from another state, the records from the prior state can be utilized for the first year of attendance to school only.
NEW Preschool Students: Preschool students not enrolled in the prior academic year are required to have a physical examination on file by the start of the new school year
Kindergarten: physical examination with updated immunization records, dental examination and vision examination performed by licensed medical professionals within one year prior to the start of the school year
2nd Grade: Updated dental examination is required to be on file prior to May 15th of the current school year.
6th Grade: Physical examination with updated immunization records and dental examination are required to be on file
9th Grade: Physical examination with updated immunization records and dental examination are required to be on file
12th Grade: All 12th grade students are required to provide proof of the meningitis vaccine. Two doses are required between 6th and 12th grade, or one dose if administered after the age of 16.

Medication

Whenever possible, medication should be administered in the home. At times when medication must be given during the school day, parents are responsible to ensure that all medication, along with written orders from a physician, be turned in to the Health Care office or the school office as soon as the child arrives at school. Medication must be brought to the school in the original package.

In order for a child to receive medication during school hours, the parent or guardian must complete a medication disbursement label. Please see someone in the health office for a label. Whenever feasible, the child should self-administer medication under adult supervision.

Athletes are considered those in the middle school and high school space that play in school matches throughout the year.
For each SCS student-athlete, a physical examination form, signed and dated by a licensed physician, physician’s assistant, or nurse practitioner, must be current and uploaded into the student’s health portal in order for the student to try out, practice, or compete in athletics at SCS
Physical examinations are good for 365 days from the date the physician completes and signs the exam.
Athletes have one year PLUS an additional 30 days from the date of their last physical to complete the sports physical requirement. The 365-day policy was created to give parents a buffer for scheduling and insurance coverage purposes.
Athletes should schedule their annual sports physical starting on day 365 from the date of their last physical and before day 395.
Athletes without a current physical on file will not be allowed to try out, practice, or compete beginning on day 395 and until a current physical is uploaded.
We strongly encourage all students who will (or might) participate in interscholastic athletics to get a physical exam in late May or June if possible.
IHSA physical examination compliance will be monitored by the Athletic Office.
All physical examination forms will be kept in the student’s health file and approved by the School Nurse.
These forms are completed by parents and uploaded onto the SNAP Health Portal before the beginning of the school year or athletic season.
